Research work of Propaedeutics of Internal Diseases

The main direction of the research work of the Department of Propaedeutic of Internal Diseases is the development and implementation of the clinical and immunological system of differential diagnosis of «norm' and «pathology' of the immune system in patients with bronchial asthma and coronary heart disease; development and implementation of methods for individual drug correction of immunity disorders in patients with bronchial asthma and ischemic heart disease.

The objectives of this study are:

  • To reveal the patterns of changes in the concentrations of cytokines IL4 and IFN-γ in the blood serum during periods of exacerbation and remission of the bronchial asthma and coronary heart disease.
  • To reveal the features of apoptosis of peripheral blood lymphocytes depending on the period of bronchial asthma and coronary heart disease.
  • To develop methods of drug correction of immunity disorders in patients with bronchial asthma and ischemic heart disease.

The following methods are used in the process of work:

  • Clinical;
  • Laboratory (immunological parameters of cellular immunity, apoptosis of T-lymphocytes of peripheral blood, determination of serum concentration of cytokines - IL4, IFN-γ, tumor necrosis factor-α, determination of the level of sodium-uretic peptides);
  • Study of the function of external respiration;
  • ECG at rest and with stress tests, Holter ECG monitoring;
  • EchoCG and Stress EchoCG;
  • X-ray and Computer tomography;
  • Atomic absorption spectrophotometry for the determination of trace elements in blood plasma and hair;
  • Statistical.

As a result of the research carried out, two doctoral and four candidate dissertations were defended. The results of scientific research have been introduced into the work of medical institutions in the form of instructions approved by the Ministry of Health of the Republic of Belarus («Using extracorporeal hemocorrection in combined basic therapy of bronchial asthma' and «Using the features of the somatic constitution for individual prediction of the effectiveness of therapeutic plasmapheresis in patients with bronchial asthma»).

These instructions were prepared based on the research results of the head of the department, professor V.P. Tsarev (a doctoral dissertation was defended, a monograph was written and three patents for invention were obtained in co-authorship: «Bioaffin sorben for selective release of immunoglobulin E»; «Method for differential diagnosis of moderate and severe course of bronchial asthma'; «Method for individual prediction of therapeutic the effectiveness of plasmapheresis in a patient with bronchial asthma»).

Two Ph.D. were defended on the topic of drug correction of immunity disorders in bronchial asthma (assistant Zh.V.Antonovich and assistant N.D. Rudenya, an instruction was issued, approved by the Ministry of Health of the Republic of Belarus, «Features of pre-treatment and ways of correcting reproductive disorders of women with bronchial asthma»). Patent No. 14554 for the invention of Zh.V. was received by Zh.V.Antonovich in co-authorship: «A method for assessing the effectiveness of the inflammatory process with the control of the course of bronchial asthma».

According to the methods of drug correction of immunity in bronchopulmonary patients, Professor E.A. Dotsenko and associate professor I.L. Arsentieva carry out several researches. The research results are confirmed by 4 patents for invention in co-authorship: «Method of hyposensitization to citrus fruits of a patient with allergic dermatitis or eczema» - No. 14635; «Method for the treatment of food allergies, manifested in the form of neurodermatitis» - No. 11548; «Method of hyposensitization of a patient with bronchial asthma to a food allergen» - No. 11023; «Method for the treatment of hormone-independent bronchial asthma» - No. 9954

The results of studies on apoptosis of peripheral blood lymphocytes and assessment of inflammation in bronchial asthma were reported at the International Congress on Respiratory Diseases (Moscow, 2002, Professor V.P. Tsarev) and at the 21st Annual Congress of the European Respiratory Society (Amsterdam ( Netherlands), 2001, J.V. Antonovich).

The department conducts research under the scientific supervision of Professor E.A. Dotsenko on the topic: «Hyperbaric oxygenation: experimental justification and use in cardiological practice.» A doctoral dissertation was defended on this topic and a monograph was written (D.N.Salivonchik).

The results of studies about hyperbaric oxygenation in acute coronary syndrome are included in the database of the Cochrane Library (Great Britain, Oxford).
